INSURANCE Information
At Michigan Maxillofacial Surgery and Implant Center, we make every effort to provide you with the finest surgical care and the most convenient financial options. To accomplish this, we work side-by-side with you to maximize any insurance reimbursement you may be entitled, for covered procedures. We work with numerous Insurance Companies,(Delta, Met Life, Cigna, BC & BS, Aetna, Medicare, Guardian and others), both in-state and from around the country. Please inquire if we participate with your particular insurance policy. While the determination of oral surgery insurance coverage is not an exact science, the front desk staff has extensive experience with insurance coding and billing and, in most cases, will be able to provide a reasonable estimate of patient responsibility. Please provide our office with all your insurance information for us to maximize your coverage.We will contact your insurance company before your scheduled surgery appointment to get a breakdown of your benefits. Please understand, however, benefits quoted over the phone, and/or even through a pre-treatment estimate, are not a guarantee of payment. Actual benefits will be determined by your insurance provider when the services are complete and a bill is submitted to your insurance company for payment. Therefore, the amount payable for services may be reduced due to circumstances including but not limited to:

The participant’s eligibility status having changed

Services performed by other providers were completed and submitted for payment

Frequency limits or annual maximum were reached after the benefits were verified or pre-treatment estimate was approved

Unusual circumstances related to a specific procedure based on insurance determinations
Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
If the exact patient payment responsibility is necessary, a predetermination process needs to be submitted, where your insurance company will provide you and us, the exact payment expected from your insurance provider, thereby giving you a better idea of your coverage in writing. If you request a predetermination, we will be happy to file the forms and will schedule surgery once we receive the notice from your insurance company.
All financial obligations must be taken care of on the day of service. Please keep in mind that the fee you pay prior to complete insurance payments or fees quoted as your portion of the surgical fee, is only based on an estimation of your benefits, and is not a guarantee of total patient responsibility. You are responsible for any amount not covered by your insurance. We will file your insurance as a courtesy. If they pay any additional amount we will send you a refund check after any additional payment is received.
For your convenience, we accept cash, checks, and all major credit cards. We also work with Care Credit for further patient financing. Care Credit offers a variety of payment plans depending on your needs and financial qualifications. All financing arrangements must be completed prior to the scheduled surgical procedure.
